My Connected Community's approach is holistic:
we work with you and the people who are important to you.

Our services are mobile, which allows us to work flexibly across environments:
home, school, workplace, and the community.

We know every person has individual strengths, interests, and needs.

​Our approach is person-centred and strengths-based
​to ensure every person is acknowledged and celebrated,
and in this way we continue to grow our practice to be neuroaffirming. 




CAPACITY BUILDING SUPPORT
We offer direct therapeutic services to assist individuals strengthen their skills, and achieve their goals.
An example of capacity building supports can include, but is not limited to:
Daily Living Skills
 Self-care, daily routines, strengthening independence, community access, financial management skills.
Social & Emotional
Emotional awareness and emotional regulation techniques,
engagement in leisure activities, social understanding communication.​
Employment and Transitions
Support to plan and transition between new workplace, home, or education settings.
Support to build employability skills and seek paid or volunteer roles. Assistance to develop micro-enterprises. ​

CAREGIVER & SIBLING SUPPORTS

Working and liaising with families, service providers, allied health professionals, and community members to enhance the choice, social inclusion, and quality of life for people living with a disability.

Helping families and caregivers strengthen their understanding regarding the impact of disability

​on a person's daily life.

​Supporting caregivers to implement strategies and tools to assist daily functioning and developing independence.

BEHAVIOUR SUPPORT
Please note: we are not registered providers for Positive Behaviour Support Plans.

Our approach to understanding behaviour and providing support is a holistic evidence-based approach
with the primary goal of increasing a person’s quality of life
and providing support to explore and understand the function of the behaviour. 

If appropriate, our behaviour support may involve a comprehensive assessment and the development of support strategies that focus on addressing the person’s needs, their environments, and overall quality of life. 
The Developmental Educators' approach is about working with individuals, their families and carers
​to develop a shared understanding about behaviours.
​
We work with you to understand behaviours and their role in communication, so that positive strategies can be developed ​to meet unmet needs and support positive outcomes for all.

FUNCTIONAL ASSESSMENTS
My Connected Community is able to offer functional assessments.

 A functional assessment measures the impact of disability and environment against an individual's ability to perform
independently across all daily living domains. 

A functional assessment can assist with understanding the support needs of an individual and developing individualised goals. ​Results from the assessment can be utilised to develop reports and recommendations.

Functional assessments may include a combination of direct interaction and observation,
interviews with key people and involved parties, and a review of relevant reports.


Functional assessment shifts the focus from the cause of disability or cognitive impairment to the impact of the condition. It's important to distinguish between capacity limitations that result directly from a person's health status and those limitations in which a person's participation is restricted due to environmental factors, including their physical environment and social circumstances
​(World Health Organization 2003).

SCHOOL & COMMUNITY
Our holistic approach includes ensuring strategies and supports are
​consistently implemented across all environments.

This may require the Developmental Educator to work directly with school/workplace staff or community members to strengthen their knowledge of disability, the related impact, and further understand how
inclusion and contribution can be promoted at all times.

TELEHEALTH SERVICES AVAILABLE
My Connected Community remains flexible with service delivery to meet everyone's needs.
We offer telehealth support which may include videoconferencing, emails, and phone calls.
